Qualifications: Care Coordinators must possess a minimum of a B.A. or B.S. degree in social work, psychology, or a related field with at least two (2) years of clinical intervention experience in serving youth with SED or emerging adults with mental health conditions. All Bachelor-level and unlicensed care coordinators must be supervised at a minimum by a licensed mental health professional (e.g., LCSW, LPC, LMFT). Ability to create effective relationships with individuals of different cultural beliefs and lifestyles.

Duties & Responsibilities/Principal Activities:
• Ensure that all phases and activities of the High-Fidelity Wraparound Process are done with high quality and fidelity and are accountable to the family team.
• Plan and facilitate the monthly Care Planning Team Meetings, in partnership with the family, natural support, informal support, and formal support.
• In partnership with the FSS, provide face-to-face direct service to families in their home or community, as needed, in implementing High Fidelity Wraparound
• Develop and coordinate the Plan of Care and Crisis Plan with the family team.
• Provide outreach and engagement to families upon referral to the program by conducting initial phone contact and the intake process with the assistance of the Family Support Specialist (FSS)
• In collaboration with the FSS, assist the family in developing a list of natural and professional supportive individuals they want to attend the Care Planning Team Meetings
• Ensure that the wraparound process is family-driven and youth-guided and that families experience ongoing participation in and ownership of their care plans!
• In collaboration with FSS, ensure coordination of service delivery with family/youth.
• Linkage to community resources, clinical services, and natural support for the child/youth and family.
• Participate in team-based supervision meetings
• Maintain up-to-date documentation and other records for each youth/family.
• Must have a valid driver’s license and proof of current auto insurance
• Other duties and tasks as assigned
